New Jersey Vital Statistics offers three quick request options through a service called "VITAL CHEK:"

1. You may call toll-free 24 hours a day, seven days a week at 877-572-6342. Requests may be billed to your major
   credit card.

2. A 24-hour FAX line, 609-392-4292 is available for vital record requests. Your FAX transmission must include the
   following information:
   • Name, date and place of vital record event.
   • If requesting a birth record - name of father, maiden name of mother.
   • Major credit card number, with expiration date.
   • Your telephone number plus the FAX number.
   • Return address.
   • Vital records are NEVER FAXED. Your certified record request will be shipped via regular mail or Federal
     Express. Indicate which method you prefer.
